In This Article You Will Learn

There are four routes into CBT-I in the UK: NHS Talking Therapies, NHS-commissioned digital programmes, private self-pay digital programmes, and 1-on-1 therapy with a qualified insomnia therapist.
No route needs a GP referral. All four accept self-referral or direct booking, so the real question is which one fits your presentation rather than who will let you in.
Digital programmes suit mild-to-moderate insomnia. Sleep anxiety, medication tapering, or a digital programme that did not work are all reasons to choose 1-on-1 therapy instead.

Table of Contents

In the UK, adults can enrol in a CBT-I programme through four main routes: NHS Talking Therapies self-referral, NHS-commissioned digital CBT-I via Sleepio or Sleepstation where your Integrated Care System has commissioned it, private self-pay digital CBT-I, or direct booking with a 1-on-1 UK insomnia therapist listed on cbti.directory or a member of the British Sleep Society. NICE’s guidance on digital CBT-I (HTG624) describes referral for face-to-face CBT-I as best practice where insomnia symptoms aren’t likely to resolve on their own, and NICE advises that sleeping tablets be used only for short periods (TA77).

Who this page is for

You are looking for how to actually start a CBT-I programme in the UK. Not how CBT-I works, not whether it works, but which door to walk through and what the first two weeks look like.

The four UK enrolment routes

Route 1: NHS Talking Therapies (free, longest waiting list)

  • Self-refer through the NHS Talking Therapies service finder. No GP referral needed.
  • Waiting times vary by area, typically 4 to 12 weeks to a first appointment.
  • Not all services offer protocol CBT-I; many deliver general CBT with insomnia adaptations.
  • Best for: mild-to-moderate insomnia, adults without medication complications, willing to wait.

Route 2: NHS-commissioned Sleepio or Sleepstation (free where commissioned)

  • Sleepio: NHS access depends on your Integrated Care System (ICS).
  • Sleepstation: NHS referral available in commissioned ICSs.
  • Enrolment is typically 5 to 10 days from referral to first session.
  • Best for: mild-to-moderate insomnia, adults comfortable with fully digital or clinician-monitored digital delivery.

Route 3: Private digital CBT-I (self-pay, fastest digital option)

  • Sleepio and Sleepstation are both available on a self-pay basis.
  • Enrolment takes anywhere from the same day to 5 days.
  • Best for: mild-to-moderate insomnia, adults who cannot access NHS commissioning or want to start immediately.

Route 4: 1-on-1 with a qualified UK insomnia therapist

  • Search cbti.directory for UK-listed CBT-I therapists.
  • Check British Sleep Society (BSS) member listings.
  • Book a free consultation call with each therapist you shortlist.
  • Enrolment is typically 1 to 3 weeks from consultation to first working session.
  • Best for: people who ‘just want it sorted’ because 1-1 is highly efficient, for moderate-to-severe insomnia, high sleep anxiety, sleep-medication use, or prior digital CBT-I that did not fully resolve things.
